CENTER FOR INDEPENDENCE OF THE DISABLED IN NEW YORK “CIDNY”Nursing Home Transition & Diversion WaiverRegional Resource Development Specialist (RRDS)The New York State Department of Health (DOH) contracts with community-based agencies throughout NYS to implement and administrator the Nursing Home Transition and Diversion Waiver (NHTDW). CIDNY’s program improves access to information, supports and services for people in need of nursing home transition or diversion for all five boroughs of New York City. Working as part of a small team, the RRDS:• Establishes collaborative relationships with community-based provider community that serves people with multiple disabilities who qualify for the Waiver and recruits and enrolls providers of services to be provided to participants under the auspice of this waiver (e.g. case management and independent living skills training, etc.) under the direction and supervision of the Lead RRDS. Maintains collaborative relationships with other Regional Resource Development Center staff.• Develops and organizes training and technical assistance for waiver stakeholders as mandated by the Department of Health. Also provides additional trainings regarding waiver eligibility and service components, person-centered planning techniques, housing and housing-related quality initiatives, independent living philosophy with an emphasis on self-determination, and related issues that have an impact on discharge planning or diversion of people from institutional placement. • Schedules and conducts outreach activities targeting discharge planning staff at nursing homes and rehabilitation facilities. Outreach activities should result in strong linkages with inpatient and long-term care providers to facilitate community re-entry.• Reviews documents (Initial/Revised Service Plans and Individual Service Reports) for evidence of person centered planning and assures that complete application packet is present including PRI/SCREEN, rights and responsibilities and other documents as outlined in the NHTDW Manual.• In the absence of Lead RRDS is able to sign and issue Notice of Determination.• Assures full compliance with project’s reporting and documentation standards. Provide data to DOH as part of quarterly reports and as directed by Lead RRDS. Participate in quarterly meetings.Qualifications: Proven experience working with people with multiple disabilities. Familiarity with the concepts of self-determination and person centered planning. Extensive experience in entitlements and government benefits. Master’s degree preferred, BSW with experience may be substituted. Extensive clinical assessment background. Proven interpersonal, presentation and written communication skills. Computer proficiency in Microsoft Office applications. Bilingual preferred. Salary: Mid 50s.
